Foveo Healthcare A Professional Medical Corporation (“we,” “us,” or “our”) is committed to protecting the privacy of your health information. This Notice of Privacy Practices describes how we may use and disclose your Protected Health Information (“PHI”), our legal duties regarding your information, and your rights concerning your health information.
Our Legal Duties

For example, we may share relevant information with another healthcare professional involved in your care.

We may use or disclose your health information to obtain payment for healthcare services we provide.
For example, we may provide information to your health insurance plan to obtain payment for services.

We may use or disclose your health information for healthcare operations necessary to operate our practice and provide quality care.
Examples may include quality assessment, staff training, auditing, compliance activities, and business management.

Uses and Disclosures Requiring Your Authorization
Certain uses and disclosures of PHI require your written authorization.
Except where otherwise permitted by law, we will obtain your written authorization before using or disclosing your PHI for purposes that require authorization, including certain uses of psychotherapy notes, certain marketing activities, and other uses or disclosures for which an authorization is required by law.
You may revoke an authorization in writing at any time to the extent permitted by law. Revocation will not affect information that was already used or disclosed based on the authorization before it was revoked.
Psychotherapy Notes
If we maintain psychotherapy notes that are subject to the special HIPAA protections for psychotherapy notes, we will obtain your authorization for uses and disclosures of those notes when required by law.
Your Rights Regarding Your Health Information
You have the following rights regarding your PHI, subject to certain limitations and exceptions under applicable law:
Right to Inspect and Obtain a Copy
You generally have the right to inspect and obtain a copy of your health information maintained by us.
Right to Request an Amendment
You may request that we amend health information that you believe is incorrect or incomplete. We may deny your request in certain circumstances permitted by law.
Right to Request Restrictions
You may request restrictions on certain uses or disclosures of your health information.
We are not required to agree to every restriction request, except where applicable law requires otherwise.
Right to Request Confidential Communications
You may request that we communicate with you about your health information by alternative means or at an alternative location.
We will consider reasonable requests as required by applicable law.
Right to an Accounting of Disclosures
You may request an accounting of certain disclosures of your PHI made by us, subject to limitations and exceptions under applicable law.
Right to a Copy of This Notice
You may request a paper copy of this Notice at any time. You may also access the current Notice on our website.
Right to Complain
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
You will not be retaliated against for filing a privacy complaint.
California Privacy Protections
California law provides additional protections concerning medical information and patient privacy. Depending on the circumstances, California law may provide rights or protections in addition to those provided by federal law.
We will comply with applicable federal and California privacy laws governing medical information and Protected Health Information.
Notice Regarding Substance Use Disorder Records
Where applicable, certain records relating to substance use disorder treatment may be subject to additional federal protections under 42 U.S.C. § 290dd-2 and 42 CFR Part 2.
Where applicable to our services and records, we will comply with the requirements governing such information.
